It is a fairly typical Portuguese municipal flag, with the coat of arms centred on a plain blue field.

Shield barry wavy of Argent and Azure, a fess Sable charged with a castle tower Or with port aand windows Gules, flanked by two wine grapols Or. Mural crown Argent, with four visible towers (town rank) and white scroll with black upper case letters reading "CONCELHO DE CASTELO DE PAIVA.
Meaning:
The name means "Castle of Paiva" and Paiva is the name of a river. This would account for the arms meaning since blue waves on white is the usual Portuguese heraldry symbolizing of rivers (while green waves on white symbolize the sea).
